| Background: Rheumatic valvular heart disease remains prevalent in Egypt and the most prevalent lesion as detected by echocardiography was mitral regurgitation. This study's objective was to evaluate prognostic value of left ventricular global longitudinal strain in mitral valve (MV) surgery for sever rheumatic mitral regurgitation (MR) and assess the cut off value of LV GLS for prediction of dysfunction post mitral valve replacement. Methods: This was an observational study that included patients with severe rheumatic mitral regurgitation in need for mitral valve replacement and were admitted at cardiology and cardiothoracic department, Benha University hospital. Results: The receiver operating characteristic analysis further established the predictive value of LV GLS, with an area under the curve (AUC) of 0.846 (p -17.9% did not show significant improvements in these parameters. This study showed that 37.5%patients developed eventful cardiac event, 25.6% of them developed cardiac mortality, 62.8% of patients were rehospitalized for HF, and 11.6% of them had repeated the surgery. Conclusions: our study demonstrates that GLS is a useful predictor of cardiac events in patients undergoing mitral valve surgery for severe MR. Patients with GLS > -17.9% are at higher risk of adverse cardiac events and may benefit from more aggressive post-operative management. | ||
